Evidence of an association between dietary intake and prostate cancer is conflicting. We examined the association between dietary fat intake and prostate cancer risk in the Prostate, Lung, Colorectal and Ovarian (PLCO) Cancer Screening Trial, a large randomized controlled trial of screening for prostate cancer, lung cancer, colorectal cancer, and ovarian cancer. Data for the present analysis were derived from diet questionnaires, collected at baseline, and from medical records, collected at baseline and 5 years later. We used Cox proportional hazards regression models to evaluate the association between intake of fat and various types of fat and risk of prostate cancer. We also evaluated the relation between postdiagnostic diet and the risk of biochemical recurrence, which is an indicator of metastatic or lethal disease. A total of 6,007 men with prostate cancer were followed for a median of 5.2 years; 742 died of prostate cancer. We found no statistically significant association between intake of total fat and risk of prostate cancer. However, we found a statistically significant inverse association between intake of total fat and risk of prostate cancer in men who consumed 1.2 g or more of alcohol per day (HR 0.83, 95% CI 0.68 to 1.00, P = 0.05) and a statistically significant positive association between intake of total fat and risk of prostate cancer in men who consumed less than 1.2 g of alcohol per day (HR 1.28, 95% CI 1.00 to 1.64, P = 0.05). There was no statistically significant association between intake of cholesterol and risk of prostate cancer. No statistically significant association was found between intake of dietary fat or specific types of fat and risk of biochemical recurrence. Our results suggest that consumption of dietary fat does not increase the risk of prostate cancer. However, the absence of a statistically significant association does not rule out the possibility of a small increased risk.Q:
